Start Date : September

Contract Length : As needed

Shift Details : 7am - end of surgical days Schedule : M-F

Schedule : Typically, 8-hour days. Can be as long as 12 hours plus and as short as 6 hours depending on case volume

Support staff : CRNA

Patients per day : 8-16

Patient population / age : 6 mo. -86 y / o

Department description : Ear, nose & throat 2 x week / spine cases 2x week / ortho 2-3x week

EMR : paper

Hospital bed size : 1 OR ASC - 2 pre-op / 3 PACU beds

Skills required : Intubations / Regional blocks Facility procedures : Lumbar / Cervical Spine decompression, hemi-discectomy, total discectomy, knee & shoulder arthroscopies

Practice Setting : Ambulatory Surgery Center

Board Certification : NBCRNA - Required

Active TX License - Required

DEA : No

Certifications required : ACLS / BLS / PALS

Minimum experience required : 3-5 years

Credentialing Timeframe : 90 Days or less if in a pinch. Can do emergency credentialing

Physician will be credentialed for 3 yrs in their system
