We're seeking a dedicated Licensed Practical Nurse (LPN) to provide hospice care for patients in Thurston County. Care is delivered in diverse settings, including private homes, adult family homes, assisted living communities, and skilled nursing facilities. Under the direction and supervision of an RN, the LPN supports the hospice plan of care and collaborates with the interdisciplinary care team to meet the physical, emotional, and spiritual needs of patients and their families.

Follow all hospice policies, procedures, and care protocols.Implement care plans under the direction of the RN and IDG team.Provide assigned nursing care, treatments, and preventive procedures.Monitor patient conditions and report changes to the RN and physician.Educate and support patients and families on care needs and symptom management.Assist in evaluating care effectiveness and adjusting plans as needed.Maintain accurate and timely clinical documentation.Attend staff meetings, IDG conferences, and required training.Participate in on-call rotations, including nights, weekends, and holidays.Communicate schedule changes and contribute to interdisciplinary care planning.Assist with procedures, maintain equipment, and observe proper infection control.Support patient and family self-care techniques.Qualifications :
Current Washington State LPN license in good standingActive BLS certificationValid driver's license and reliable transportationMinimum 1 year nursing experience in hospice or home health preferredMinimum 2-3 years of healthcare related experience requiredProficient with EMR systems and healthcare technologiesAbility to exercise sound judgment, initiative, and work independently within scope of practiceAdaptable to diverse social, cultural, and religious backgroundsIf you're a compassionate and skilled LPN who finds purpose in patient-centered care, we'd love to hear from you.
