Occupational Therapist OT Home Health (PRN)

Empath Home Health a member of Empath Health is seeking a Home Health OT Occupational Therapist. The Home Health OT Occupational Therapist provides occupational therapy services related to patients level of function and performance of daily living.

Locations : home health visits available in west Manatee county (including Bradenton Anna Maria Island Holmes Beach Bradenton Beach Palmetto etc.)

Schedule : PRN 10 visits per week

The Home Health OT Occupational Therapist :

• Provides treatment to the patient in compliance with agency policies and procedures and federal and state regulations.
• Performs initial and on going assessments to determine level of functioning and patients ability to perform activities of daily living including OASIS assessments at appropriate time points.
• Develops and revises a plan of care in consultation with the physician and other care team members which will enable patient to achieve realistic goals of self-care.
