Responsibilities :

• The responsibility and accountability to handle all referrals within the skilled nursing facilities
• Manages those coming into the building in the sub-acute unit
• Manage referrals via multiple communication platforms (fax phone and online)
• Collaborate and communicate with nursing professionals as well as insurance companies to manage insurance verifications and prior authorizations
• Continual review and authorization of discharge planning ordering supplies and setting up referrals etc
• Manage a strong relationship and partnership with Nursing Leadership
• Assist on the unit ass needed

Pay Range : $34.20 - $48.45

Pay is based on experience skills and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
