Summary
Design and modify groups and programming to meet the current patient needs.Monitor patient volume and work with Business Development to promote the services offered. Develop appropriate treatment plan. Communicate with referral sources and other appropriate parties regarding patient progress in accordance with HIPAA regulations. Develop and conduct didactic and other appropriate groups. Coordinate patient discharge planning and implementation including aftercare. Assist in the development and attainment of outpatient department goals and objectives. Provide services to patients in various modalities including group and / or individual therapy sessions. Facilitate aftercare services. Maintain accurate reports of patient volume and identified quality indicators. Provide educational programs and information to the community regarding treatment options. Identify coverage issues with regard to insurance and authorization. Complete and maintain accurate records with regard to utilization review. Maintain regular communication with office staff regarding schedule. Complete documentation and charges in a timely manner. Education and Experience Master’s degree in social work or related field with experience working with patients with substance abuse and related diagnoses. Certificates, Licenses, Registrations Must have LCAC and LCSW; or LCAC and equivalent of LCSW. (LMFT or LMHC) NOTE : This position is required to work Monday, Tuesday and Thursday evenings from 5 pm to 8 : 30 pm for CDIOP coverage. The remaining hours are very flexible.
