Your Key Responsibilities :
- Attend and contribute to interdisciplinary team meetings, providing assessments as needed.
- Perform initial and ongoing assessments to evaluate participants' functional abilities and rehabilitation needs.
- Recommend adaptive equipment and home modifications to enhance safety and independence for participants.
- Collaborate to develop, implement, and revise individualized treatment plans based on patient progress.
- Provide physical therapy interventions across various settings including clinics, homes, and contracted facilities.
- Educate participants and caregivers on safe equipment use, home safety strategies, and daily living skills.
- Maintain timely and accurate documentation of all assessments and treatment plans.
- Actively participate in daily team meetings, contributing to effective care planning.
- Supervise and mentor Physical Therapy Assistants, ensuring compliance with care plans.
- Implement staff training on physical therapy best practices and equipment usage.
- Support quality improvement and compliance initiatives within our organization.
- Perform other interdisciplinary duties as needed, maintaining professionalism.
- Stay up-to-date with licensure and certifications through ongoing professional development.
- Ensure confidentiality of Personal Health Information (PHI) according to guidelines.

Degree from an accredited physical therapy program, meeting the American Physical Therapy Association standards.Successful completion of the national licensing examination and possession of a valid state license.Three years of physical therapy experience in various settings preferred.Experience working with geriatric or frail populations is advantageous.Strong communication skills to collaborate with clinical and non-clinical staff.Ability to multitask and prioritize effectively in a dynamic environment.Competency in reading and interpreting technical instructions for patient care.Skilled in observing and assessing patients through various cues.Adaptable to changing patient needs and care settings.Must possess a valid driver's license and auto insurance.Current CPR / BLS certification is required.Successful completion of drug screening and background checks.Up-to-date immunizations (TDP, MMR, Varicella, Flu, Pneumonia) and a recent physical exam.Must meet health and physical requirements for safe patient care.Why Choose Interim HealthCare?
