Registered Nurse – Home Health (Full Time)

A respected home health provider is seeking an experienced and compassionate  Registered Nurse (RN)  to join its growing clinical team. This is a full-time field position serving patients across the  greater Cincinnati area , delivering high-quality, personalized care right in the comfort of their homes.

About the Role

As a Home Health RN, you’ll work closely with physicians and an interdisciplinary care team to ensure every patient receives exceptional, evidence-based care. You’ll have the independence of home visits combined with the full support of a collaborative leadership team that understands the realities of clinical work.

Key Responsibilities

Coordinate with physicians to manage medications and update plans of care

Supervise LPNs, aides, and therapy staff to maintain consistent, high-quality patient outcomes

Perform comprehensive assessments, OASIS documentation, recertifications, and discharges using an EMR system

Help reduce hospital readmissions through proactive case management and early intervention

Participate in team meetings, continuing education, and orientation programs

Qualifications

Active  Registered Nurse (RN)  license in good standing within the state of Ohio

Current CPR certification

Reliable transportation, valid driver’s license, and proof of auto insurance

Working knowledge of  Medicare / Medicaid regulations  for Home Health

Strong communication, organization, and patient-advocacy skills
