Position OverviewThe Occupational Therapist is responsible for evaluating patients in their homes and developing individualized therapy plans that help restore daily living skills, independence, safety, and functional mobility.You will work closely with patients recovering from illness, injury, surgery, or chronic conditions to improve their ability to perform everyday activities.Key Functions & ResponsibilitiesPatient Evaluation & AssessmentConduct comprehensive in-home occupational therapy evaluationsAssess patient ability to perform Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs), Assist patient with hygiene while educating and training caregivers and loved ones on assisting with bathing and self-care.Review physician orders, medical histories, and diagnosesIdentify functional deficits and establish patient-centered goalsTreatment Planning & ImplementationDevelop individualized treatment plans tailored to each patient's functional needsImplement therapeutic interventions to improve strength, coordination, balance, and independenceTrain patients in adaptive techniques to improve daily living skillsRecommend and train patients on the use of adaptive equipment or assistive devicesPatient & Caregiver EducationEducate patients and caregivers on safe mobility, home safety, and fall preventionProvide instruction on energy conservation techniques and body mechanicsDevelop personalized home exercise and functional training programsProgress MonitoringMonitor and document patient progress toward therapy goalsAdjust treatment plans based on patient response and outcomesCommunicate regularly with physicians and the interdisciplinary care teamCollaborationWork closely with nurses, physical therapists, physicians, and case managersParticipate in care coordination to ensure comprehensive patient careProvide recommendations for continued therapy or discharge planningDocumentation & ComplianceMaintain accurate and timely patient documentationComplete all required records for billing, compliance, and insuranceEnsure compliance with HIPAA regulations and agency policiesService AreasWe are currently seeking therapists to serve patients throughout Maricopa County, including:• West Valley• Paradise Valley• North Scottsdale• Central Phoenix• Peoria• GlendaleQualifications✔ Active Arizona Occupational Therapist (OT) License✔ Home health experience preferred but not required✔ Strong clinical assessment and problem-solving skills✔ Excellent communication and patient education abilities✔ Ability to work independently and manage a field caseload✔ Passion for helping patients regain independence and quality of lifeJoin the Inspired TeamAt Inspired Home Health Care, we believe the best patient care starts with supporting our clinicians.
