Role Overview
The Physical Therapist will deliver rehabilitative therapy services to patients within a skilled nursing and rehabilitation environment. This role focuses on helping individuals improve mobility, manage pain, restore function, and achieve long-term independence through personalized treatment plans.
Key Responsibilities
- Perform comprehensive patient evaluations to determine therapy needs and functional goals.
- Develop and implement individualized treatment plans in accordance with clinical guidelines and facility standards.
- Provide therapeutic interventions including strengthening, mobility training, balance activities, gait training, and pain management.
- Maintain accurate and timely documentation for evaluations, daily notes, progress reports, and discharge summaries.
- Collaborate closely with interdisciplinary staff including nursing, physicians, and other rehabilitation professionals.
- Educate patients and caregivers on treatment plans, safety strategies, and home exercise programs.
- Ensure compliance with all regulatory, clinical, and ethical standards.
- Participate in team meetings, care planning, and ongoing performance improvement initiatives.
Qualifications
Active state licensure as a Physical Therapist (PT).Experience in skilled nursing or long-term care preferred.Strong communication, problem-solving, and clinical assessment skills.Ability to work independently and manage a varied caseload.Commitment to providing high-quality, patient-centered care.
