Position : Physical Therapist (PT)

Shift : Day Shift (08 : 00 AM - 04 : 30 PM)

Shift Length : 8 hours

Contract Duration : 13 weeks

Role Overview

The Physical Therapist will deliver rehabilitative therapy services to patients within a skilled nursing and rehabilitation environment. This role focuses on helping individuals improve mobility, manage pain, restore function, and achieve long-term independence through personalized treatment plans.

Key Responsibilities

• Perform comprehensive patient evaluations to determine therapy needs and functional goals.
• Develop and implement individualized treatment plans in accordance with clinical guidelines and facility standards.
• Provide therapeutic interventions including strengthening, mobility training, balance activities, gait training, and pain management.
• Maintain accurate and timely documentation for evaluations, daily notes, progress reports, and discharge summaries.
• Collaborate closely with interdisciplinary staff including nursing, physicians, and other rehabilitation professionals.
• Educate patients and caregivers on treatment plans, safety strategies, and home exercise programs.
• Ensure compliance with all regulatory, clinical, and ethical standards.
• Participate in team meetings, care planning, and ongoing performance improvement initiatives.
