The ideal candidate will be responsible for delivering exceptional nursing care to our patients with both psychiatric and medical needs in a home setting. This will include completing a comprehensive psychiatric / medical assessment and recommendations on plan of care to patients medical provider. This role will involve working closely with patients, families, and interdisciplinary teams to develop and implement effective care plans.
