Job Title : Registered Nurse

Experience : Minimum 1 full year of Adult Home Health or Hospice experience

License : Must have a valid nursing license in hand

Certifications : CPR, BLS

Must-Have : Proficient in HomeCare HomeBase and Oasis, valid driver's license and auto insurance, reliable transportation

Job Description :

• Provide, coordinate, and direct home nursing care based on physician orders and agency policies.
• Apply the nursing process competently to ensure effective patient care.
• Supervise and evaluate Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.
• Make home visits and attend periodic office meetings.
• Handle cases including Start of Care, Wound Care, and Follow-up Visits.
• Comfortable driving to patient homes daily and covering a mileage radius of up to 75 miles from the home clinic.
• Collaborate with Clinical Manager and attend weekly meetings at the clinic.
• Meet weekly productivity points of 30, which equates to 40 hours per week.
• Complete all documentation within 24 hours of patient appointments to ensure effective billing and follow-up care.
• Work on-call one weekend every 5 weeks, varying by location.
• Open to floating to nearby clinics if patient census is low to meet financial needs.
• Maintain a professional dress code with scrubs and demonstrate proficiency in using EMR systems, including HomeCare HomeBase and Oasis.

Completion of a retraining course may be required if EMR system proficiency needs updating within the last 6 months.
