Position Summary
Pine Acres Rehabilitation & Care Center is seeking a Full-Time Physical Therapist (PT) to join our dedicated in-house rehabilitation team. The Physical Therapist is responsible for evaluating, planning, and delivering individualized therapy services to residents recovering from illness, injury, or surgery. This role plays a vital part in helping residents restore mobility, improve strength and balance, and maximize independence in daily living.
Key Responsibilities
Conduct comprehensive physical therapy evaluations to determine patients’ mobility and functional needs.
Develop and implement individualized treatment plans that promote strength, endurance, balance, and overall mobility.
Provide direct therapy interventions in accordance with physician orders and facility protocols.
Educate patients, families, and caregivers on exercise programs, safety, and adaptive techniques to support continued progress.
Collaborate closely with interdisciplinary team members—including OT, SLP, nursing, and social services—to ensure coordinated, high-quality care.
Maintain accurate and timely documentation in the electronic medical record system.
Participate in care plan meetings, clinical discussions, and ongoing professional development.
Adhere to all applicable state, federal, and facility policies, procedures, and professional standards.
Qualifications
Required : Doctorate or Master’s degree in Physical Therapy from an accredited program.
Required : Current Iowa Physical Therapist license (or eligibility).
Preferred : Experience in subacute, skilled nursing, or long-term care rehabilitation settings.
Excellent communication, clinical reasoning, and teamwork skills.
