Job Posting : Children's Behavioral Health Case Worker
Location : 34 E. State Street, Mt. Morris, NY
Employment Type : Full-Time 35-40 Hours a week
Salary : $18.00-$19.00 an hour
General Description
Perform skill building functions that support the development of a child / teen’s life and social skills, working one on one with each participant in his / her own home and in the community. Facilitate family members’ understanding of the child / teen’s needs as related to his / her condition / diagnosis.
Essential Duties and Responsibilities
- Provides a meaningful “core” service for each child / teen assigned, which includes face to face visits per month based on need and acuity level, to ensure all care plan goals are addressed to the satisfaction of the clients served.
- Works directly with families to help them understand their child’s condition and subsequent needs.
- Demonstrates emotional confidence and good judgement to engage children and families.
- Manages complex behavioral needs of children while facilitating skill building activities, continually assuring the safety of the children and families, and applying appropriate emergency and crisis intervention principles and techniques.
- Provides prevocational services including teaching skills, building appropriate work habits, completing applications, role playing interviews, etc., as needed.
- Provides various supported employment services to assist youth with maintaining employment including but not limited to “on the job” support.
- Enters required documentation of every interaction with client into Electronic Health Record system to facilitate timely Medicaid billing.
- Provides LPHA Driven and Family and Youth informed skill building activities to caseload assigned, maintaining accurate and verifiable documentation of all services rendered.
- Using a person-centered approach, completes comprehensive and detailed assessments, reviews, crisis plans, and care plans as required by standards set by DOH and lead Health Home contractors.
- Demonstrates, promotes and practices cultural competency towards clients through respect and understanding.
- Recognizes and embraces diversity through supporting and encouraging the strengths of each individual youth and family served.
- Assists youth and families in the acquisition and maintenance of public benefits e.g., Medicaid, SNAP, Social Security, etc.
- Provides linkage / referrals and coordination of resources / support for medical care, mental health, social, recreational, and housing / basic needs, etc.
- Remains current in the field, support and implement best practice service models within the program.
- Makes independent decisions and recommendations regarding immediate risk factors presented and provides crisis intervention and follow-up as needed.
- Provides education to youth and family concerning the nature and desired effect of services.
- Assists youth and families in managing and planning for safety needs by assisting with implementation of previously developed individualized safety / support plans.
- Utilizing a team approach, maintains accessibility for clients during the workday, while demonstrating flexibility to be available to clients and families outside of normal business hours as needed.
- Serves as client advocate.
- Participates as a member of Youth’s Care Team to carry out individualized and strength-based plan of care through regular case conferences and case reviews.
- Participates in staff meetings and other CFTSS / HCBS department activities.
- Ensures compliance with all applicable local, State, Federal and Health Home Network regulations and agency policies.
- Participates in mandatory ongoing training and professional development offered.
- Represents Catholic Charities on appropriate boards and committees as assigned by CC Director.
- Completes other duties as assigned.

Requirements : Qualifications :
Bachelor’s degree in human services or related field and two years of paid experience working with persons with disabilities and / or chronic illnesses.Bachelor’s degree in an unrelated field with five years of paid experience working with persons with disabilities and / or chronic illnesses.Bilingual (English / Spanish) skills may be required for some positionsRelevant combination of education and experience will be considered
