Home Health LPN PRN

Family Home Health Network , an Affiliate of the  Alden Network , is a provider of intermittent Home Health care, palliative care, hospice care, and skilled / custodial private care services throughout the Chicagoland area.

We are seeking a dedicated Home Health nurse with a strong commitment to quality patient care. To succeed in this role, you must possess excellent clinical nursing and customer service skills. You will be responsible for completing a detailed initial assessment of the patient, the family, and the home environment to determine specific home health care needs, monitor and regularly re-evaluate the patients nursing needs, and initiate the appropriate plan of care as well as making necessary revisions as patient status and needs as they change.

Minimum Qualifications :

• Licensed Practical Nurse in the state of Illinois
• Home Health experience preferred
• Current CPR
• Valid IL driver’s license and auto insurance policy
• HCHB EMR experience a plus
• Must possess excellent clinical nursing and customer service skills
