This role involves working closely with patients to develop individualized care plans, provide education about wound management, and collaborate with a multidisciplinary team to improve patient outcomes.

Responsibilities

• Conduct comprehensive assessments and evaluations of patients with acute and chronic wounds.
• Develop and implement individualized treatment plans based on best practices and evidence-based guidelines.
• Perform wound debridement, dressing changes, and other wound care procedures.
• Monitor patient progress and adjust care plans as needed to promote healing.
• Educate patients and their families on wound care management, including proper dressing techniques and preventative measures.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure continuity of care.
• Document all patient interactions, assessments, and treatment plans in compliance with healthcare regulations.
• Stay informed about the latest advancements in wound care and integrate them into practice as appropriate.
• Participate in quality assurance initiatives and contribute to the enhancement of wound care protocols.
