Job Description :
The Part-Time Admission Specialist plays a key role in managing and coordinating the admissions process by handling incoming leads, supporting referral navigation, and ensuring a smooth transition from inquiry to enrollment. This role supports both the client and the internal team through timely communication, documentation, and follow-up.
Key Responsibilities :
Lead Intake & Management :
- Receive and manage all incoming leads via calls, emails, texts, website inquiries, fax, and employee referrals.
- Promptly respond to leads and provide initial guidance and information about services.
Referral Routing & Coordination :
Route referrals to appropriate service pathways including Private Pay, VA, Medicaid Waiver (EDWP, Structured Family Caregiving, NOW / COMP).Complete and submit necessary referral forms accurately and in a timely manner.Medicaid Eligibility Support :
Assist referrals with accessing Medicaid eligibility assistance resources and services.Provide guidance on what documentation is needed and help connect them to the right contacts or agencies.Personalized Care Planning :
Assist in initiating individualized care planning in coordination with the care team, ensuring referrals receive a person-centered approach.Admission Preparation :
Support the preparation process for client admissions to the day center by ensuring all forms, documentation, and necessary steps are complete.Coordinate with internal teams to ensure a smooth admission transition.Pipeline Management :
Document all referral activities and lead updates in Salesforce CRM.Maintain consistent follow-up with leads until admission is achieved.Customer Service & Support :
Conduct follow-up calls with newly admitted clients to ensure satisfaction and gather feedback post-admission.Team Communication :
Communicate effectively with internal team members, including :Transportation team
Day center staffCorporate support teamReporting & Documentation :
Complete and submit weekly reports on lead activity, referral pipeline, and admission progress.Requirements :
Experience in healthcare intake, admissions, customer service, or referral coordination preferred.Strong communication and interpersonal skills.Highly organized and detail-oriented.Proficient in Salesforce CRM or similar platforms.Ability to manage multiple tasks and prioritize in a fast-paced environment.Compassionate and client-focused mindset.Position Details :
