Behavioral Health Case Manager

We are seeking a compassionate and client-oriented Behavioral Health Case Manager to join our interdisciplinary clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court-Ordered Treatment In this role, you will play a vital role in supporting individuals experiencing mental health and / or substance use challenges.

You will conduct comprehensive assessments, develop individualized care plans, coordinate access to resources, and advocate for your client’s well-being.

Your work will directly contribute to improving the lives of our clients and their families.

Responsibilities :

  • Assists clients with identifying treatment and recovery goals and provides supportive services to help clients reach their goals.
  • Completes assessments, crisis plans, and individualized service plans.
  • Works collaboratively with the clinical team to engage, educate, communicate, and coordinate care with members, their families, behavioral health providers, medical and dental providers, community resources, and others to ensure that all services prescribed in the individualized service plan are implemented.
  • Conducts outreach and follow-up of services, including, but not limited to, crisis interaction, missed appointments, and hospital discharges, to ensure adequate resources are available and in place.
  • Performs all case management functions associated with the caseload, including completing the assessment and service planning processes and identifying the need for further or specialty evaluations.
  • Pursues best practice outcomes for people with mental illness, including continuing education, employment, independent housing, and community tenure.

Requirements :

  • GED / High school diploma with a minimum of 1.5 years of behavioral health experience, or,
  • Computer Skills : Microsoft Word, Excel, Outlook, electronic medical records, and NextGen are preferred.
  • Valid AZ Driver License : Must have a valid Arizona driver’s license, be 21 years of age with a minimum of 4 years of driving experience, and meet the requirements of the driving policy.
  • Must have a valid Arizona Fingerprint Clearance card (Level 1) or apply for an Arizona Fingerprint Clearance card within 7 working days of employment.
  • Interpersonal skills that allow for professional and positive relationships with co-workers, members, families, and other community members
  • Good communication skills, evidenced by a non-judgmental approach, effective listening, and positive interactions.
  • A basic understanding of the DSM-V is preferred.
  • A certified Case Manager is strongly preferred.
  • Must pass a background check, TB test, education verification, and other pre-employment screenings.
