Social Worker MSW

This is a part time position working in the Case Management department at Northwest Medical Center.

Summary :

Provides assessment and crisis intervention in acute care settings; plans, organizes, and implements social work services for patients and families.

Provides long term care planning for patients / families. Responsible for complex discharge planning. Intervenes as needed with hospital patients with mental health problems or substance abuse problems.

Provides assessment with case management for pediatric patients.

Position Qualifications / Minimum Requirements

Education :

Required : Masters of Social Work degree

Experience :

Required : Two (2) years acute clinical experience

Skills :

• Able to handle crisis situations that require judgment critical to patient welfare; Appropriate prioritization and planning skills, while achieving safety and quality in a high volume environment;
• Engaging and effective communication and service skills; Adherence to policy and procedures; Process improvement oriented;

Self-initiative to assist with whatever is needed.

Licenses / Certifications :

For Skilled Nursing Unit : Fingerprint Clearance Card
