Key Responsibilities
Perform outreach to members and provide support with care plan next steps and resources
Serve as the front-line support for member and provider inquiries, explaining care plan procedures
Document and maintain non-clinical member records in accordance with state and regulatory requirements
Required Qualifications
High School diploma or GED required
1 - 2 years of related experience required
