Description
The Health Navigator helps to drive increased engagement and member adherence for key quality program measures. This includes managing multi-modal forms of communication including phone, sms, email and portal. This role collaborates with other teams within the organization to deliver care to members in a coordinated and streamlined manner. This role may also interface with providers, health plans and other partners on an as needed basis.

Conduct outreach to members with specified clinical gaps to help coordinate care and gap closure activitiesSchedule appointments for patients with the internal team and / or external provider officesManage work through multiple portals required to access all relevant patient dataConduct outreach to provider offices in efforts to help patients schedule appointments, relay relevant health information, or obtain medical recordsManage sms and email queues as assigned for incoming and outgoing patient communicationAble to communicate with members in a language with or without the use of translation servicesManage basic office functions such as faxing, voicemail, and mailAble to accurately enter patient data into relevant platforms and document outcomes in a clear and concise mannerAble to educate patients on basic healthcare terminology and topicsPre-visit prep : assist with confirmation calls, rescheduling, and chart prepPost-visit work : assist in closing out any visit orders, such as referrals, faxing of notes, sending lab referrals, etcAssist patients in completing health questionnaires and documenting responses within EMR and / or designated platform.Maintain all patient data in compliance with the organization’s PHI practices and following Health Insurance Portability and Accountability Act (HIPAA) standardsOther duties as assignedQualifications
Must reside in California to fulfill occasional onsite requirementsMust be fluent in Mandarin, Cantonese, or SpanishMinimum of high school diploma or GED; Associate's degree or higher preferredAt least one (1) year of healthcare related experienceReliable transportation and willingness to travel to office locations and provider sites as neededAbility to keep a high level of confidence and discretion when dealing with sensitive informationAbility to prioritize and effectively multi-taskExcellent organizational skills and strong attention to detailExcellent analytical critical reasoningEffective interpersonal / professional communication skills with patients, peers, providers, health plans or other internal / external colleaguesExcellent customer service skillsAbility to work independently with limited supervisionProficient in Microsoft Office (Excel, Word, Outlook, PowerPoint, etc.)You are great for this role if :

The total pay range for this role is $22 - $25 per hour. This salary range represents our national target range for this role.This role follows a hybrid work structure where the expectation is to work at home and onsite as needed on a weekly basis. This position will be primarily remote with occasional travel to provider offices in the San Gabriel Valley area. The home office is located at 1600 Corporate Center Dr., Monterey Park, CA 91754.Preferred working hours : Tuesday - Saturday : 8am - 5pm or Monday - Friday 10am to 7pm
