Position Summary
We are seeking a compassionate and organized Licensed Social Worker to join our healthcare team. The Social Worker plays a vital role in supporting residents’ psychosocial needs, care planning, discharge planning, and compliance with state and federal regulations.
Key Responsibilities
Complete psychosocial assessments and ongoing evaluations for residents
Develop, implement, and update individualized care plans
Assist residents and families with adjustment to illness, placement, and care transitions
Coordinate discharge planning and community resources
Provide counseling and crisis intervention as needed
Advocate for residents’ rights and ensure dignity and quality of life
Maintain accurate documentation in accordance with state, federal, and facility requirements
Participate in care conferences and interdisciplinary team meetings
Ensure compliance with Minnesota Department of Health and CMS regulations
Qualifications
Licensed Social Worker (LSW) in the State of Minnesota
Bachelor’s degree in Social Work (BSW)
Experience in a skilled nursing facility, long-term care, or healthcare setting preferred
Knowledge of Medicare, Medicaid, and long-term care regulations
Strong communication, documentation, and organizational skills
Ability to work collaboratively with residents, families, and care teams
