A company is looking for a Community Health Worker to support patients with complex chronic conditions through advocacy, resource connection, and care coordination.
Key Responsibilities
Screen patients for Social Determinants of Health needs and connect them with appropriate community resources
Coordinate care by facilitating communication between patients and their healthcare providers
Provide wellness coaching and assist patients in developing practical life skills
Required Qualifications
Community Health Worker Certification, CMA, RMA, or an Associate's degree in health sciences or related field with 3+ years of healthcare experience
Experience in identifying community resources and assisting patients in accessing them
Experience working within an interdisciplinary healthcare team
Comfortable advocating for patients with internal and external stakeholders
Prior experience with complex patients and / or underserved populations
