POSITION SUMMARY

The Medication Technician is responsible for monitoring the medication program and to oversee all activity in the medication room. Provide a high level of customer service for both residents and team members. This is accomplished though promoting CRISTA Senior Living Service Standards. Oversee residents' self-medication and assist the Resident Staff Manager or Health and Wellness Director in training new medication technicians and provides mentoring and shadowing. Support the caregiving staff as needed with care needs and transfers.

ESSENTIAL FUNCTIONS

Medication Management:

  • Perform medication set-up and provide medication assistance to residents with the self-administration of their medications, ensuring safe, accurate, and timely assistance in accordance with CRISTA policies and applicable WAC requirements.
  • Observe and practice the Seven "Rights" to Medication Administration when assisting a resident with self-administration of medication.
  • Notify the Resident Staff Manager and Health and Wellness Director if there are concerns about a resident's ability to self-manage medications.
  • Assure residents receive their medications as prescribed and in a timely manner.
  • Order and receive medications promptly and notify family members or responsible parties when medications need to be picked up to ensure they are available for residents and prevent shortages.
  • Notify the Resident Staff Manager and Health and Wellness Director, when applicable, if medications are not delivered to the community on time.
  • Count and document all controlled substances in the Narcotic Book at the end of each shift in accordance with established procedures. Immediately report any discrepancies to the Resident Staff Manager or Health and Wellness Director before leaving the shift.
  • Accurately transcribe provider-authorized medication orders into the eMAR and submit medication orders to the pharmacy promptly in accordance with CRISTA policies, established procedures, and applicable WAC requirements.

Medication Documentation:

  • Documents medication assistance in the electronic health record (EHR) using PointClickCare (PCC) electronic Medication Administration Records (eMAR) immediately following each medication assistance, ensuring accurate, timely, and compliant documentation in accordance with CRISTA policies and applicable WAC requirements.
  • Document the reason for PRN medication administration and the medication's effectiveness in the eMAR.
  • Accurately document all medication assistance in the eMAR and maintain all required medication records, including CSRs and narcotic logs.
  • Conduct daily audits of resident eMARs, Centrally Stored Records (CSRs), and Pharmacy Transaction Records to ensure documentation is complete, accurate, and compliant with CRISTA policies and applicable WAC requirements.
  • Keep medication files and resident records current and up to date.
  • File information in the appropriate section of Resident Care file once issue is complete, (i.e. medication has been ordered and received, Resident Staff Manager and Health and Wellness Director has reviewed home health agency report and orders, etc.).

Resident Care:

  • Document daily resident care and services in PointClickCare (PCC) in accordance with CRISTA policies and applicable WAC requirements.
  • Adhere to all medication administration policies and procedures.
  • Assist residents in making doctor's appointments and travel arrangements to and from appointments.
  • Take resident vital signs, Capillary Blood Glucose (CBG) and all RN delegated duties in accordance with WAC regulations.
  • Perform apartment visits and apartment checks on a regular basis.
  • Report any significant incidents on the appropriate forms and to the Resident Staff Manager, Health and Wellness Director or Executive Director.
  • Assist residents with ADL tasks including housekeeping tasks, making beds, serving meals, assisting with personal laundry, assisting with showers, assisting with toileting, dressing, undressing, oral care, hair care, transfers and ambulation, showers and any other needs addressed on the resident service plan.
  • Communicate changes in residents' conditions accurately and efficiently to RN/LPN, RSM and HWD, emergency personnel, physician and resident family/responsible party.

Resident Safety:

  • Assist in keeping the environment safe and clean for all residents.
  • Keep all medications in their appropriate storage area.
  • Keep medication cupboard, carts, and doors always locked, when not in use.
  • Wash hands per policy and follow proper infection control procedures.
  • Observe residents for safety, changes in condition, potential abusive situations, etc., and report any changes of condition or other pertinent information to supervisor.
  • Follow safety protocols and procedures, making sure safety is the primary concern for all residents and team members.
  • Maintain possession of the assigned PalCare communication device throughout the shift and demonstrate proficiency in its proper use. Remain accessible and responsive to alerts, calls, and other work-related communications.
  • Respond to emergency signal/call system.
  • Keep the medication room clean and well organized, free of any personal belongings, food and drink not directly used for resident medication assistance.
  • Be contact person for emergency personnel during resident emergency.
