Hospital Pharmacist - Laredo, TX
The hospital pharmacist is responsible for overseeing and coordinating all pharmaceutical care for patients at our facility. This position reports directly to the Director of Pharmacy and works closely with physicians, nurses, and other healthcare professionals as part of an interdisciplinary team.
- Core responsibilities include :
- Managing drug therapy protocols to ensure safe, appropriate, and effective medication use. Conducting thorough reviews and evaluations of medication orders to identify and prevent potential drug-related problems.
- Providing drug information and pharmaceutical education to medical staff, other healthcare professionals, patients, and pharmacy students. Staying up-to-date on latest developments in pharmacological therapies.
- Documenting all medication interventions and outcomes to support continuous quality improvement of drug therapy regimens.
- Verifying accuracy and completeness of medication orders prior to order entry. Double-checking order accuracy after filling by pharmacy technicians.
- Maintaining working knowledge of pharmacy information systems, automation, and software applications.
- Performing other duties as assigned by pharmacy leadership.
