Job description
Position Summary The Case Manager coordinates patient care across the continuum, ensuring appropriate utilization of resources, medical necessity compliance, effective discharge planning, and financial stewardship. This role collaborates with the interdisciplinary team to remove barriers to care and facilitate safe, timely transitions to the next level of care while meeting regulatory and payor requirements. Key Responsibilities Conduct utilization reviews for Medicare and non-Medicare patients to ensure medical necessity and compliance with QIO standards. Develop, implement, and manage individualized plans of care in collaboration with physicians and the interdisciplinary team. Identify barriers to care and coordinate hospital and community resources to support patient and family needs. Facilitate safe, timely discharge planning and appropriate referrals. Ensure accurate and timely documentation in the medical record per policy and state regulations. Manage insurance authorizations, certifications, and payor communications to support reimbursement. Collaborate with clinical services to ensure documentation supports reimbursement guidelines. Monitor contract compliance and support financial risk reduction efforts. Promote patient and employee safety in alignment with organizational safety standards. Participate in team meetings, family conferences, and facility-wide initiatives. Qualifications Current licensure as required by state regulations (RN or Social Worker, as applicable). Experience in case management, utilization review, discharge planning, or acute/post-acute healthcare setting preferred. Knowledge of medical necessity criteria, insurance authorization processes, and reimbursement guidelines. Strong communication, organizational, and interdisciplinary collaboration skills. Ability to manage multiple priorities in a fast-paced healthcare environment.
