Job Description
ZRG Partners has been retained to recruit a Primary Care Nurse Practitioner for the Center for Elders' Independence (www.cei.elders.org) in the San Francisco Bay area.
This is an outstanding opportunity to join a rapidly growing PACE (Program of All-Inclusive Care for the Elderly) organization that uses a highly collaborative interdisciplinary team approach to care planning and implementation for the purpose of providing high quality, affordable, integrated health care services to the elderly.
Nurse Practitioners at CEI are empowered providers who prioritize building strong relationships with their participants and their families , spending substantial time with each individual, with a manageable participant load ranging from 50 to 70 .
CEI’s Nurse Practitioners report directly to their Chief Medical Officer, Dr. James Mittelberger, a nationally recognized physician leader who has dedicated his career to the care of vulnerable patients across the continuum of care from hospital to home.
