Acclaim is seeking a Full-Time Advanced Practice Provider to join our Infectious Disease / HIV team. The Department of Internal Medicine provides specialty services across the continuum of care at JPS. We currently have over one hundred providers who work in various locations including the Main Hospital, Ambulatory Surgery Center, Community Medicine Clinics, and a robust Street Medicine program.
This position is grant funded and subject to re-evaluation based on program changes.
The Department of Internal Medicine utilizes a robust APP onboarding program,inclusive of a 6-8 week training phase in which APPs can safely learn and drill the fundamentals of patient care, before their official patient templates open. Furthermore, this onboarding includes a thoughtful and well-calibrated template ramp up, such that APPs have the time to learn and grow appropriately in their new role.

Job Summary :
This position is an outpatient service for routine care for those patients who are HIV+ or at risk for HIV and any related health issues consisting of assessment, diagnosis and the development and implementation of patient-specific treatment plans for patients and their families.
While the schedule is typically Monday-Friday, the exact dates and times will be developed in conjunction with the candidate and the needs of the program.
The clinician will order and interpret laboratory and diagnostic tests, as well as consult with other members of the healthcare team when appropriate. Additional services include providing cervical cancer screenings and assisting / performing high resolution anoscopies.

Facilitates the timely flow of patients via collaboration with interdisciplinary team members and supports all interdisciplinary relationships.
Recognizes acute / emergent situations requiring immediate responsiveness and initiates life-saving interventions, alerting the interdisciplinary team members for assistance.
Recognizes and incorporates age-specific requirements, communicates plan of care with patients and families.
Advocates for patients and family members in educational needs, treatment options, participation in care and ethical issues and patient rights with recognition and consideration of the diverse needs of the individual / family.

Duties and Essential Job Functions :
Coordinates the multidisciplinary care delivered to all patients, to optimize patient care.
Reviews patient medical charts to diagnose, treat, and provide care management and education to patients and families.
Makes appropriate patient referrals as necessary.
Monitors guidelines for the management of care and for ensuring the services are delivered in compliance with the guidelines established by JPS Health Network and Acclaim Physician Group.
Provides evidence-based and compassionate care to all patients as assigned to improve patient outcomes and increase patient satisfaction scores. Demonstrates patient centered and patient valued behavior.
Maintains a collaborative environment with other referring physicians, residents, clinical staff, and support staff to provide quality patient care.
