As a Case Manager youll play a vital role in delivering client-centered trauma-informed care . Youll coordinate essential mental health services collaborate with an integrated care team and ensure clients receive evidence-based interventions that foster growth and healing.

Responsibilities

• Educational & Executive Support : Empower students by conducting education assessments creating personalized success plans and leading dynamic daily executive functioning activities that spark academic and cognitive growth.
• Client Monitoring & Safety : Be a trusted guide by scheduling check-ins tracking attendance managing return-to-school documentation and performing daily emotional wellness check-ins to ensure client safety and well-being.
• Clinical Coordination & Facilitation : Play a key role in the treatment processcollaborate on individualized plans facilitate IOP / PHP sessions manage schedules and support smooth admissions and discharges.
• Family & Team Collaboration : Build strong connections by working closely with parents clinical teams and schools to share progress updates resolve challenges and create effective discharge plans.
• Administrative & Documentation : Keep everything running smoothly by maintaining accurate reports participating in treatment team meetings and teaching essential life skills that help clients thrive.
