Medication Reconciliation Pharmacy Technician
Department : Pharmacy Services
Reports to : Pharmacy Manager
About the Role
We are seeking a detail-oriented Medication Reconciliation Pharmacy Technician to join our pharmacy team. In this vital role, you will work under pharmacist supervision to obtain and document accurate medication and allergy histories from patients or family members during the admission process. Your efforts will directly contribute to patient safety and continuity of care.
What You’ll Do
Conduct patient interviews in the Emergency Department or inpatient units to collect comprehensive medication histories.
Compile and document all home medications, including prescriptions, OTC drugs, supplements, and devices (e.g., patches, pumps).
Use pharmacy records, provider communication, and EHR tools (e.g., Cerner RX External History) to verify and reconcile medication lists.
Identify discrepancies and communicate critical issues to pharmacists or providers.
Educate patients and caregivers on maintaining an accurate medication list.
Accurately enter medication data into the electronic health record (EHR).
Mentor and assist in training new technicians.
Support general inpatient pharmacy operations as needed.
What We’re Looking For
Education : High school diploma or equivalent.
License / Certifications :
Active Indiana Pharmacy Technician license.
Certified Pharmacy Technician (CPhT) credential required.
BCLS (CPR) certification if providing direct patient care.
Experience : Minimum 2 years of pharmacy technician experience (retail / outpatient preferred).
Other Requirements :
Must pass the national pharmacy technician certification exam within 6 months of hire (maximum 3 attempts allowed).
Must be at least 18 years old.

Work Environment & Physical Demands
Hospital setting with frequent patient contact and potential biohazard exposure.
Requires standing, bending, and lifting up to 40 lbs.
