JOB TITLE : Transitioning Youth Specialist
STATUS : Full-time
SALARY : $22.00 - $24.00 per hour
LOCATION : Charles, Calvert, St. Mary's, and Anne Arundel County areas, as assigned
Job Purpose :
The Transitioning Youth Specialist at The Arc Southern Maryland will play a vital role in supporting youth with intellectual and developmental disabilities (IDD) as they transition from school-based services into adult life. This position will ensure the provision of tailored services to help individuals achieve successful outcomes in areas such as employment, higher education, community involvement, and independent living.
Key Responsibilities :
- Case Management : Develop and implement individualized transition plans for youth with disabilities, working closely with students, families, educators, and external service providers.
- Coordination of Services : Act as a liaison between families, schools, government agencies, and community organizations to ensure the seamless delivery of services, including job coaching, post-secondary education options, and vocational training.
- Advocacy : Advocate for youth with disabilities to ensure access to necessary resources, supports, and opportunities for personal growth.
- Workshops & Training : Organize and conduct workshops for youth and their families on topics related to employment, life skills, and accessing community resources.
- Program Development : Collaborate with the CDS and Development teams to develop innovative programs and services that address the unique needs of transitioning youth.
- Resource Development : Identify and secure resources and services that support successful transitions, including funding, partnerships, and new initiatives.
- Documentation & Reporting : Maintain up-to-date case notes, transition plans, and progress reports in compliance with organization and funding requirements.
- Other duties as assigned.

Assists individual(s) to meet his or her employment goals by providing work force readiness training and building on individual strengths and capabilities.Coordinates with Arc job coaches to ensure that individuals are matched with opportunitiesDevelops and / or assists with the development of relationships within DORS and the school systems.Assists individual(s) to plan, make informed choices, follow through on responsibilities and advocate for themselves.Uses effective, sensitive communication skills to build rapport and channels of communication by recognizing and adapting to the range of individual(s) communication styles.Demonstrates the ability to communicate clearly and concisely, both orally and verbally.Learns and uses terminology appropriately, explaining as necessary to ensure individual(s) understanding.Maintains accurate records and submits records to appropriate sources in a timely fashion.Maintains standards of confidentiality and ethical practiceAccepts and employs the agency's core values.Attends community meetings (e.g. Chambers of Commerce, civic meetings) as neededAttends CDS team meetings as neededMust be able to work a varied schedule in both Southern MD and Anne Arundel County areas, as assigned.Possesses the ability to pass the criminal background check mandated by the State of Maryland.Qualifications :
Bachelor's degree preferredMinimum of two years of experience working with youth with intellectual and developmental disabilities, ideally in a transition or case management role required.Knowledge of local, state, and federal services and benefits available to youth with disabilities (e.g., DORS, Social Security, Medicaid).Strong organizational and time-management skills, with an ability to manage multiple cases and tasks simultaneously.Excellent communication skills, both written and verbal.Ability to work collaboratively with diverse stakeholders, including families, schools, and community partners.Familiarity with the Americans with Disabilities Act (ADA), Individuals with Disabilities Education Act (IDEA), and other relevant legislation.Reliable transportation and a valid driver's license are required.AMERICANS WITH DISABILITY SPECIFICATIONS (PHYSICAL DEMANDS)

Work Environment :
The position requires working within schools, community settings, and the office environment. The ability to travel throughout Southern Maryland and Anne Arundel County is required. Some evening or weekend hours may be necessary for events and workshops.
