- Start of Care : 2.2 - 2.5 Points Per Visit
- Discharge : 1.0 - 1.5 Points Per Visit
- Resumption of Care / Recertification : 1.5 - 1.75 Points per Visit
- Revisit : 1.0 Point per Visit
