Social Services Designee - Full Time

Shift : 8 : 00am - 4 : 30pm

Job Description : PATIENT POPULATION :

The patient population served in the DP / SNF is 21 years old and older. This also includes services which affect facility staff, physicians, visitors, vendors and the general public.

POSITION SUMMARY :

Under the supervision of the DPSNF Director , the Social Service Designee is responsible for performing a variety of routine discharge planning tasks and other social services to patients and residents of the hospital, primarily in the DPSNF. The Designee may also act at the direction of a Social Service Assistant, with oversight by the LCSW.

Must be able to work normal / scheduled working hours to include Holidays, call-backs, weeknights, weekends, and on-call. Agrees to participate, as directed, in emergencies and community disasters during scheduled and unscheduled hours. As a designated disaster service worker you are required to assist in times of need pursuant to the California Emergency Services Act.

EDUCATION / TRAINING / EXPERIENCE :

Sub-Acute - To perform this job successfully, an individual should be a high school graduate or equivalent, preferably a Certified Nurse Assistant (CNA). Must have at least 2 years of experience / knowledge of Title 22 and Minimum Data Set (MDS) . Must have at least 2 years prior experience in facilitating discharge planning, working with convalescent facilities, durable medical equipment (DME), home health, etc.

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence if required.

Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, and percentages if required.

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

To perform this job successfully, an individual should have basic typing and computer skills.

LICENSURE / CERTIFICATIONS :

To perform this job successfully and individual must have and maintain current BLS certification or obtain within orientation process pior to working in the department.Must possess a Social Service Designee certification or completed within 30 days of hire or possess an associate degree in social work or other psychosocial degrees.

Responsibilities and Essential Functions :

• Indicates Essential Function
• Collaborates with interdisciplinary team to identify and address social and emotional needs of all residents / patients. This includes discharge planning evaluation for each resident.
• At the direction of the Director of the DP / SNF, participates actively in care planning, MDS, interdisciplinary team, and other activities as directed designed to facilitate regulatory compliance of the DP / SNF.
• Maintains and utilizes a comprehensive community resource file to meet specific needs, i.e. specialized equipment, family counselors, home health agencies, welfare, etc.
• Ensures that residents and families are informed of their rights upon admission, and assist the healthcare care to honor those rights while residents are under our care.
• Complies with mandated documentation requirements.
• Provides an avenue for family / patient or resident complaints including theft and loss, and inform them of procedures for resolution.
