The ideal Licensed Practical Nurse (LPN) will have a strong background in patient care, medical documentation, and health coaching . This role involves working closely with patients in long-term care (LTC), memory care, and outpatient facilities .Key Responsibilities :

• Provide direct patient care under the supervision of RNs or physicians.
• Conduct health coaching sessions to promote wellness and chronic condition management.
• Perform accurate medical documentation in compliance with healthcare regulations.
• Assist in outpatient settings, LTC, and hospital medicine environments .
• Administer medications and treatments as prescribed.
• Monitor patients' vital signs and report any changes.
• Support patients undergoing dialysis and educate them about the process.
• Utilize medical terminology to communicate effectively with healthcare teams.
• Conduct diagnostic evaluations to assess patients' health status.
