Job Description :

Responsibilities :

Provide high-quality physical therapy services to patients.

Develop and implement individualized treatment plans to improve patients’ daily living skills and independence.

Work closely with an interdisciplinary care team to ensure optimal outcomes.

Maintain accurate documentation and progress notes.

Qualifications :

Active Florida PT License (required).

Prior experience in Home Health preferred.

Strong interpersonal, communication, and organizational skills.
