Purpose :
As our Integrated Care therapist on the Integrated Health Home (IHH), you will work with a collaborative interdisciplinary medical and mental health team to provide whole person care.
Job Summary :
You will be the lead therapist on the integrated team and serve as an expert consultant to the medical team. You will report to the Program Supervisor.
Profile :
Are you experienced to conduct initial comprehensive assessments of clients?
Have you provided brief, episodic psychotherapy, addressing mental health issues and physical health complaints?
Do you have experience in an integrated care environment?
Are you looking to manage a small caseload of clients needing both psychotherapy and complex case management?
Do you want to be a consultant to medical providers and conduct mental health assessments, psychotherapeutic and wellness interventions?

What we need for this job :
- Masters in Psychology, Social Work, Counseling or related field, plus 1+ year internship, and one (1) or more years experience in a clinical setting
- Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) or full licensed acquired within twelve (12) months.
- Experience in integrated or medical behavioral health
