Care Advisor

A company is looking for a Care Advisor, Care Concierge.

Key Responsibilities

Assist members with assessments and recommend service plans based on their needs

Coordinate and schedule services while following up with members through outbound calls

Support members in accessing community resources and complete administrative tasks

Required Qualifications, Training, and Education

High School Diploma preferred

Experience in Public / Community Health or related field preferred

Experience in Social Services, Care Coordination, or Healthcare Navigation is a plus

Must be 18 years of age or older

Computer proficiency and knowledge of clerical functions required
