Job Title : Registered Nurse

License : Must have license in hand

Certifications : CPR, BLS

Must-Have : Proficiency in HomeCare HomeBase (HCHB) and OASIS; Must have a vehicle, driver's license, and auto insurance

Job Description :

• Provides, coordinates, and directs the provision of home nursing care according to physician orders based on agency policies and procedures.
• Applies the nursing process competently to deliver patient care.
• Reports to the Clinical Manager and directly supervises and evaluates Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.
• Conducts home visits with periodic office meetings.
• Manages an average of 5-8 patients per day.
• Responsible for various types of cases, including start of care, wound care, follow-up visits, etc.
• Must be willing to travel up to 75 miles from the home clinic.
• Compensation for mileage follows IRS standard rates from patient home to patient home.
• Expected to achieve weekly productivity points equivalent to 30 points, translating to 40 hours per week.
• Documentation must be completed within 24 hours of the patient appointment.
• Open to potential on-call rotations and weekend requirements.
• Orientation and compliance will include a retraining course if the required systems have not been used within the last six months.
