Job Description

Job Title : Registered Nurse I

Experience : Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience

License : Must have a valid license in hand

Certifications : CPR, BLS

Must-Have :

Proficiency in HomeCare HomeBase EMR, used within the last 6 months
Proficiency in OASIS, used within the last 6 months
Reliable transportation, current driver's license, and agency-required liability insurance

Job Description :

Provide, coordinate, and direct home nursing care according to physicians' orders based on agency policies and procedures.

Responsible to the Clinical Manager.

Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.

Perform home visits and attend periodic office meetings.

Manage an average of 5-8 patient visits per day, equating to 6-8 points.

Handle types of cases such as Start of Care, Wound Care, Follow-up Visits, etc.

Complete all documentation within 24 hours of the patient appointment.

Travel up to 75 miles from the home clinic with mileage reimbursed at the IRS Standard rate.

Attend weekly meetings at the clinic and be willing to float to nearby clinics if needed to meet financial needs.

Charting proficiency required in HomeCare HomeBase and OASIS.

No weekend rotation or on-call duties unless discussed during the interview.

Accept all assigned patients, and complete weekly productivity points of 30, equating to 40 hours per week.
