Position Responsibilities :
- Overcome barriers within the healthcare system to ensure patients receive the care and treatment needed.
- Initiate communication with patients upon learning they have a suspicious mammogram or other diagnostic finding.
- Guide patients through the healthcare system, identifying any barriers to or possible barriers to care.
- Facilitate appointments and assist patients in completing forms for financial and other types of assistance. Assist with arranging for transportation when needed.
- Utilizing various administrative tools (forms and system applications) track patient intake and treatment interventions and outcomes, summarizing and reporting findings
- Develop relationships with internal and external health care providers to familiarize them with the patient navigator' role, and encourage referrals.
- Connect patients to community and social support services. Maintain a learning library.
- Educate individuals and community groups in the early detection of cancer and cancer treatment.
- Build relationships with other patient navigators.
- Completes nursing documents accurately and in a timely manner following Joint Commission standards.
- Provides patient education and documents appropriately using the approved documentation forms.
- Demonstrates the ability to identify signs and symptoms of emergencies and provides BLS management using vital machine and medication reaction kit appropriately and immediately.
- Assesses and reassesses patient complaint of pain and document findings.
- Attends annual mandatory In services. Maintain certification in Basic Life Support.
- Delegates patient care responsibilities to assigned care givers based upon patient and staff competencies.
- Demonstrates a calm, professional manner during a crisis or emergency situation ar all times within patient care environment.
Follows approved protocol in receiving critical results and performing timely and accurate communication and follow-up.
Qualifications :
Current FL Nursing License and current BLS required.Graduated from an accredited school of nursing. BSN preferred, OCN preferred.3 - 5 years experienceBenefits :
