Intake and Eligibility Screening Specialist

We're looking for a compassionate, detail-oriented Intake and Eligibility Specialist to support senior individuals applying for support services and resources. In this role, you'll guide callers through eligibility screenings, application submissions, and follow-up support. You'll also collaborate closely with agency partners to ensure smooth and timely processing of each case.

This is a great opportunity for someone who enjoys helping others, thrives in a structured environment, and is bilingual in English and Spanish / Creole.

Key Responsibilities :

• Conduct client intake and screening to facilitate access to long-term care programs
• Communicate with clients, gather information, and accurately document case details in agency databases
• Assess case urgency and route to appropriate services for timely resolution
• Maintain accurate records and update application statuses in real time
• Provide ongoing support and outreach to ensure clients remain informed throughout the process
