Job descriptionSerious and Persistent Mental Illness (SPMI) Depression and anxiety Substance use disorders Cognitive decline and dementia Complex medical and psychosocial issues Conduct initial mental health assessments and develop individualized treatment plans. Provide evidence-based therapy and mental health interventions. Travel to various medical and community settings to reach clients who may have barriers to accessing traditional in-office care. Collaborate with physicians, nurses, social workers, and other healthcare professionals. Monitor clients' progress, adjusting treatment plans as needed. Maintain accurate and timely clinical documentation. Master's or Doctoral degree (PsyD or PhD) in Psychology, Social Work, Marriage and Family Therapy, or related field. Active Minnesota license as an LICSW, LP, LMFT, or LPCC Strong clinical skills and a passion for working with geriatric populations. Commitment to culturally responsive care and integrated health models. Ability to work independently and collaboratively within multidisciplinary teams. Compassion, empathy, and dedication to improving clients' quality of life. Flexible schedules to support a healthy integration of your professional and personal life.
