Position Summary
The Case Manager 1 is an entry level position responsible for providing case management with individuals and their families to establish appropriate linkages, referrals, coordination of care, and to provide regular follow-up on needed services and supports under the supervision of a qualified case management supervisor. The Case Manager 1 will identify and assess client needs and evaluate, coordinate, and ensure necessary services are provided.
Required Education and Experience
- Bachelor's degree from an accredited college or university in an applicable human services field (including social work, counseling, behavioral science, psychology, or a related field.
- No professional experience required - less than one year of human service experience. Prefer some relevant experience.

Knowledge of the nature of serious mental illness developmental disabilities, and substance abuse depending on the population served.Ability to observe and report accurately on the functioning of individuals and families.Ability to engage and sustain ongoing relationships with individuals receiving services.Ability to learn and acquire the knowledge and skills necessary to perform all case management services to clients.Ability to handle confidential information appropriately.Must possess time management skills.Ability to effectively use active listening skills. May be called upon to travel to any Horizon worksite, transport clients, provide services to clients at other Horizon locations.
