Case Manager 1

Hours : Full-time - 37.5 hours per week (Monday through Friday, 8 : 30 a.m. - 5 : 00 p.m.)

Location : Lynchburg VA, 24501

Compensation dependent on experience

Position Summary

The Case Manager 1 is an entry level position responsible for providing case management with individuals and their families to establish appropriate linkages, referrals, coordination of care, and to provide regular follow-up on needed services and supports under the supervision of a qualified case management supervisor. The Case Manager 1 will identify and assess client needs and evaluate, coordinate, and ensure necessary services are provided.

Required Education and Experience

• Bachelor's degree from an accredited college or university in an applicable human services field (including social work, counseling, behavioral science, psychology, or a related field.
• No professional experience required - less than one year of human service experience. Prefer some relevant experience.
