Key Responsibilities
Manage inbound and outbound calls for care coordination services
Conduct outreach to community organizations and providers for enrollment and credentialing
Perform assessments and provide resources to members and providers in real-time
Required Qualifications
Associate's degree in a health-related field or medical assistant training
Minimum 4 years of experience in care coordination
Minimum 2 years of recent experience in a clinical environment
Minimum 1 year of telephonic outreach or call center experience
