Job Description

The Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans according to each patient's needs and physician direction. This role is responsible for the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate effective community re-entry. Additionally, the physical therapist serves as an educator to patients, families, students, and coworkers while upholding the organization's mission and core values.

Responsibilities

• Complete comprehensive evaluations of assigned patients and develop appropriate treatment plans focused on patient discharge goals.
• Collaborate with the interdisciplinary team to address barriers to discharge and ensure timely training and support for patients and families.
• Manage individual, concurrent, and group treatment sessions effectively based on patient care needs.
• Document patient care clearly and accurately in compliance with relevant guidelines and organizational policies.
• Participate in the student program, providing supervision and feedback for student learning and development.
• Actively pursue professional development through continuing education and involvement in professional activities.
• Adhere to discipline-specific state practice acts and maintain professional licensure in good standing.
