RN Supervisor The Rehabilitation Center at Jupiter Gardens is in search of a capable, hard working and talented RN Supervisor to join our ever growing team.The Rehabilitation Center at Jupiter Gardens is a warm and close-knit subacute care and nursing center proudly serving the distinct needs of the Jupiter community. Responsibilities Include :

• Guides and directs and supervises the activities of the certified nursing assistants and LPNs.
• Shares responsibilities for conducting admission assessments, MDS and care plans with the Nurse Manager and the other staff nurses.
• Reviews, modifies as needed, patients’ care plans in collaboration with patient, family / designated representative and the interdisciplinary team.
• Attends care planning and discharge planning review meetings. Provides patient / family education during stay and to prepare for discharge. Ensure that the rooms / beds assigned to new admissions, In-house transfer are ready to accept the patient.
• Conducts daily rounds with the Nurse Managers. Communicates and report relevant and significant patient related information or changes in condition to the Physician, Nurse Practitioner and Administrative Nurse Manager.
• Responds appropriately to patient care emergencies, provides guidance and direction to other caregivers in managing emergency events.

Qualifications / Requirements :

Must have a current and active license in the State of Florida

Minimum 2 years of experience in the LTC industry

At least 1-2 year experience as a Shift Supervisor or Manager a plus

Must be able to effectively asses and evaluate patients / residents during the admission, transfer and discharge process

Must have some knowledge of State and federal regulations and the application of facility protocols

Must live within 15-25 miles of the facility
