Providing direct community-based behavioral health services and support for the individual and family as indicated in the Person-Centered Plan. Monitor and track consumer hours and annual intake dates and complete as assigned. Assisting the Case Manager in monitoring and evaluating the effectiveness of interventions as evidenced by symptom reduction and progress toward goals. Responsible for completing the OnTarget tasks assigned within 24 hours. Assisting the Case Manager with consulting identified non-medical providers, engaging community and natural supports and including their input in the person-center / ed planning process. Ensure that all information obtained while at Carolina Therapeutic Services is kept confidential and will remain confidential beyond employment. Ensure confidentiality training is updated. Provider Responsibilities Report critical concerns, problems, and lack of engagement to supervisor in a timely manner and actively seek Supervisor’s input and assistance to resolving the concern. Participate in multi-disciplinary staffing and meetings. Attends all appropriate in-services, workshops, or continuing education programs, which are trainings provided by CTS Health or on behalf of CTS. Responsible for completing appropriate annual intakes. Maintains strict confidentiality according to all Federal and State guidelines and requirements. Exemplifies personal and professional conduct.
