As a Licensed Master Social Worker (LMSW) at Wellstar Medical Center, you will play a crucial role in providing comprehensive social work services to our patients. Your responsibilities will include conducting assessments, developing treatment plans, and offering counseling and support to patients and their families. You will work collaboratively with a multidisciplinary team to ensure that patients receive holistic and effective care.

Key Responsibilities :

• Conduct psychosocial assessments and develop individualized treatment plans.
• Provide counseling and support to patients and their families, addressing emotional, social, and environmental factors affecting their well-being.
• Coordinate care with healthcare providers and community resources to ensure comprehensive patient support.
• Assist patients with discharge planning and transition to appropriate community resources or services.
• Advocate for patients' needs and rights, ensuring they receive the necessary support and resources.
• Maintain accurate and timely documentation of patient interactions and treatment progress.
• Participate in case conferences and team meetings to contribute to patient care planning.
