Job descriptionValid LVN License in California One year or more of experience in either Home Health or Hospice is preferred, but not required Previous experience in a hospice setting is a plus Ability to build rapport with patients Compassionate and caring demeanor Excellent written and verbal communication skills
We are seeking a full-time LVN Licensed Vocational Nurse to join our team! You will be responsible for delivering high-quality care to assigned Hospice patients. Clinical experience is preferred and candidates with Hospice experience are highly desirable.
Responsibilities
- Provide Skilled Nursing Care in a home or facility setting
- Ensure the appropriate use of meds, DME, and supplies
- Educate and support patients, families, and caregivers
- Participate in IDT and updating the Plan of Care
- Provide our patients and families the best care possible
- Daily travel is required when working
