Social Work Department Manager
Manages the overall function of the Social Work Department assuring that its policies and procedures are effectively carried out. Responsible for the management and coordination of the admission process beginning with initial inquiry, maintenance of applicant pool, admission determinations and placement. Supervises staff Social Workers, Admissions Social Worker, and Admissions Social Work Assistant. As a member of the senior leadership team, plays a key role in shaping and executing the organization's strategic direction and mission; collaborates with other leaders to ensure FSL values are infused in day-to-day operations and a philosophy of person-direct care fostering teamwork, accountability, and a culture of continuous improvement across the organization. Supports Culture of Excellence through active role across all neighborhood initiatives.
Essential Job Functions :
- Oversees Admissions Coordinator in working with other clinical staff to determine level of care and appropriateness of admission.
- Assures that all necessary information is up-to-date and reported monthly.
- Responsible for census development and maximum utilization of beds in conjunction with community need and facility occupancy goals.
- Collaborates with the Marketing team and provides oversight to marketing activities performed by Admissions Coordinator.
- Responsible for the effective management of the intake and admission process assuring that appropriate responses are made following inquiry.
- Maintains all required regulatory documentation.
- Assures that admission notes and psychosocial history are developed and recorded within appropriate time frames pursuant to the Member's needs, and the Home's policies and code requirements.
- Assists with facilitating the adjustment of Members, families and significant others through individual and group interaction.
- Oversight of the Social Workers to assures that clinical documentation is recorded in the medical record on a timely basis consistent with Member's needs, policies and procedures of the department, and codes, rules and regulations.
- Oversees Social Workers in serving as facility Discharge Coordinator; works in conjunction with other clinical staff to coordinate interdisciplinary activity leading to discharge plan. Assures that discharges are made in a timely manner and in accordance with Member's rights.
- Ensures that Social Workers utilize appropriate community resources when necessary to meet the needs of the Members; assists families and significant others with communication with agencies.
- In collaboration with Director of Life Enrichment, responsible for the development, implementation and ongoing operation of the Members' Council (Residents' Council), and assures that all Members have the opportunity to participate through an appropriate structure. Records minutes and attendance at Members' Council and apprises Administration of same.
- As requested, conducts or arranges facility in-service training on the role of the Social Worker, Members' rights and responsibilities, New York State Abuse Reporting Law, psychosocial implications of aging, institutionalization, disability, death and dying, or other related topics.
- Hires, trains, supervises and evaluates Social Work Department staff including Social Workers, Admissions Coordinator, and Admissions / Social Work Assistant.
- Assists Social Work staff as necessary to meet Members' needs and maximize the effectiveness of social work practice. Acts as back-up as needed.
- Arranges for and / or provides in-service education to Social Work staff on topics pertinent to social work practice.
- Develops department work schedule and assures coverage during times of departmental staff absences.
- Annually establishes and works to achieve approved departmental goals and objectives.
- Develops, recommends and implements approved programs, policies and procedures relevant to the Social Work Department; reviews policies and procedures on an annual basis, or more often as necessary.
- Develops and submits an annual operating and capital expense budget for the Social Work Department.
- Participates on the Quality Management Steering Committee.
- Works with other management staff to assure that service to Members and effectiveness of general operations is maximized.
- Participates in the development of marketing strategies to maximize bed utilization.
- Performs other duties and responsibilities as required by Administration from time-to-time.
Other Job Functions :
Education : Bachelor's in Social Work required, Master's Degree preferredExperience : Minimum of 5 years of relevant work experience and one-year recent experience in Gerontological Social Work practice.Skills : Must be able to meet the public and develop and deliver effective presentations to individuals and groups.Licenses / Certifications : Licensed Social Worker (LMSW or LCSW) preferred.Expectations of the Position :
