Job Description

Job Title : Registered Nurse I

Job Specialty : Home Health

Job Duration : 13 weeks

Shift : Day 5x8-Hour (08 : 00 - 17 : 00), Monday to Friday

Guaranteed Hours : No guaranteed hours

Experience : Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience required

License : Must have license in hand; pending license not accepted

Certifications : CPR, BLS

Must-Have :

Proficiency in OASIS and HomeCare HomeBase (HCHB), used within the last 6 months or completion of a retraining course if used outside of the last 6 months
Reliable transportation with a current driver's license and agency-required liability insurance
Experience in Home Health
Ability to travel up to 75 miles from the home clinic
Ability to complete documentation within 24 hours of the appointment
Comfortable with variable mileage radius that may result in longer drives between patient appointments
Open to on-call rotations and weekend work as required
Must accept every patient on schedule regardless of distance

Job Description :

Provide, coordinate, and direct home nursing care according to physicians' orders and agency policies

Supervise and evaluate Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses

Responsible for home visits and periodic office meetings

Assigned between 5-10 patients per day, equating to roughly 6-8 points

Complete documentation within 24 hours of patient appointments

Hit weekly productivity points of 30, equivalent to 40 hours per week

Opportunity to float to another nearby clinic for additional patients if needed

Attend to various types of cases, including Start of Care, Wound Care, Follow-up Visits, etc.

Attend weekly meetings and provide feedback to the Clinical Manager

Maintain professional attire, with a dress code of scrubs

Use electronic medical record systems for patient documentation, requiring proficiency in specific software such as Point care

This job requires flexibility, adaptability, and commitment to providing high-quality care in a home health setting.
